Flexible Scheduling
At Patient 1st Healthcare Services (P1HS), we provide timely, reliable visits based on your physician’s orders and Medicare home health guidelines. After your referral is received, our team coordinates quickly to complete the initial assessment and begin services as ordered. Visit frequency and disciplines (nursing, therapy, etc.) are scheduled according to your approved plan of care, and we adjust promptly when your condition changes—always keeping your physician informed.

Who is eligible for home health medical services?
Most patients qualify when leaving the hospital, nursing home, or after surgery, when starting new medications, managing wounds, or coping with a chronic condition like CHF, COPD, or diabetes. In general, eligibility also requires that you are homebound (meaning leaving home takes considerable effort and you need help, or it is medically unsafe to go out often) and that the services are intermittent and part of a physician-directed plan of care.
What should I do if I need home health medical services?
If you think you need home health services, call your doctor or hospital discharge team and request a referral to Patient 1st Healthcare Services (P1HS). You may also call P1HS directly for guidance. After we receive the referral and insurance details, we confirm eligibility, schedule an assessment, and start your physician-ordered care plan. We answer questions fast anytime! For urgent symptoms (chest pain, severe shortness of breath, sudden weakness, heavy bleeding), call 911 or go to the ER!!
Are your providers licensed or certified?
Yes. P1HS clinicians are properly licensed, trained, and screened. We verify credentials and provide ongoing competency training to ensure safe, high-quality care in the home.
